Whereas, in consideration of being permitted to attend a course for instruction in firearms or medical skills, lifesaving topics, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, agree to the following:

1) I agree to indemnify, hold harmless and defend Warlizard Tactical, LLC (including Joel Gaines, Michael Gaines, Greg Given, Ken Smith, and any agents, volunteers, RSOs and/or assistants) hereinafter referred to collectively or individually as “Instructor”, any organizations and/or their agents permitting the use of a classroom, range, building, land, and premises, from any and all fault, liabilities, costs, expenses, claims, demands, or lawsuits arising out of, related to or connected with my presence at and/or participation in the course of instruction; the use of instruction in the future; the discharge of firearms by me, the use of medical techniques or tools, lifesaving skills, my presence on or use of the classroom, range, building, land, and premises (“Premises”); and, any and all acts or omissions of myself. *I agree

2) I understand that I will be involved in dangerous activities with dangerous items. I am willingly including myself in training of a deadly, martial art. I waive for myself and for my executors, personal representatives, administrators, assignees, heirs and next of kin, any and all rights and claims for damages, losses, demands and any other actions or claims whatsoever, which I or they may have or which may arise against an Instructor or any organizations permitting the use of the classroom, range, building, land, and premises (including but not limited to my death and/or any and all injuries, damages or illnesses suffered by me or my property), which may, in any way whatsoever, arise out of, be related to or be connected with: the course of instruction; the Premises, including any latent defect in the Premises; my presence on or use of said Premises; my property (whether or not entrusted to Instructor); and the use of medical tools or techniques, the ammunition and discharge of firearms. Instructor shall not be liable for, and I, on behalf of myself and on behalf of my executors, personal representatives, administrators, assignees, heirs, and next of kin, hereby expressly release the Instructor and any organizations permitting the use of the classroom, range, building, land, and premises from any and all such claims and liabilities. *I Agree

3) I also hereby expressly assume any risk of taking part in the course for instruction in medical techniques or the use of medical tools or lifesaving skills or the use of firearms and taking part in the activities on the Premises, which include, but are not limited to, instruction in the use of firearms, the discharge of firearms and the firing of live ammunition, shooting from and around vehicles, driving, medical procedures, use of medical tools, and lay person lifesaving skills. *I Agree

4) I hereby acknowledge and agree that I have read this instrument and understand it terms and am executing this instrument voluntarily. I furthermore hereby acknowledge and agree that I have read, understand and will at all times abide by all classroom safety rules, range rules and procedures and any other rules and procedures stated by the Instructor. *I Agree

5) I expressly agree that this instrument is intended to be as broad and inclusive as permitted by law and that if any provision of this instrument is held invalid or otherwise unenforceable, the enforceability of the remaining provisions shall not be impaired thereby. No remedy conferred by any of the specific provisions of this instrument is intended to be exclusive of any other remedy, and each and every remedy shall be cumulative and shall be in addition to every other remedy now or hereafter existing at law or in equity or by statue or otherwise. The election of any one or more remedy hereunder by the Instructor shall not constitute any waiver of Instructor’s right to pursue other available remedies. This instrument binds me and my executors, personal representatives, administrators, assignees, heirs and next of kin. *I Agree

6) I acknowledge that I may be intentionally or accidentally subjected to an action that invokes pain. I understand that pain in itself is a form of training and I hold no one responsible or otherwise liable for any painful activity. I will likely hear harsh words during my training to desensitize me from those being used as a weapon against me in the future. I may experience topics or conversations that are highly uncomfortable as a part of my training.I Agree

7) I understand that photos and videos will be taken of me and my participation in this training. Warlizard Tactical LLC et al have unrestricted and perpetual license and ownership to use my likeness in the form or photos, videos, or derivatives in any way they choose in any public format, whether for commercial use or any other use. *I AgreeBlur my likenessIf you select "Blur my Likeness" you must tell us.

8) By signing up for, paying for and attending said training, I represent that there are no legal or physical restrictions or impediments with me in the handling or participating in firearms, lifesaving, or personal protection training. Also, I certify that I am not a person prohibited under federal or state law from possessing a firearm, and none of the above conditions apply to me. I have been informed of all risks involved and I voluntarily agree to the terms and conditions stated above. *I Agree
